Notice checklist

A notice should identify the copyrighted work and challenged material with a precise URL, provide contact details, and state a good-faith belief that the use is not authorized by the owner, its agent or law.

It should also state under penalty of perjury that the information is accurate and the signer owns the right or is authorized to act, followed by a physical or electronic signature.

Counter-notice

A counter-notice generally identifies affected material and its former location, provides contact details, states under penalty of perjury that restriction resulted from mistake or misidentification, includes legally required jurisdiction and service consents, and bears a signature.

Repeat infringement and accuracy

Where appropriate and technically possible, access associated with repeated infringement may be restricted. Material misrepresentations may create liability. Other safety categories should not be mislabeled as copyright notices.
